What is the process for the immigration Physical?

In the first step of the immigration process, the patient will meet with a healthcare provider in order to establish care in the form of an initial physical and receive an order for blood and urine tests at a second location to screen for infectious diseases and STDs as required by USCIS. This initial meeting with the healthcare provider will be billed to the patient’s insurance company*. Please note that the bill reflects the level of care and services provided and that abnormal findings may require greater levels of care and more services.

Once the lab results have been received by JKW Medical Group, the healthcare provider will review them to determine the next step for the patient. If the lab results report is normal, then a follow up appointment will be scheduled. At this follow up appointment, the healthcare provider will perform the final physical and mental health wellness checks and finalize the patient’s vaccine records in accordance with USCIS standards. Any additional services prescribed by the healthcare provider will be reflected in the bill. This follow up meeting with the healthcare provider will be billed to the patient’s insurance company*.

If the lab results are abnormal, additional appointments with the healthcare provider may be scheduled to address the findings before the final follow up meeting can take place. Medical bills related to these appointments will be billed to the patient’s insurance company*.

After completion of the follow up appointment, the patient will receive their sealed envelope containing signed I-693 forms required by USCIS. A $220 legal service fee will be incurred (I-693 forms, etc) upon receival of the sealed envelope. This legal fee is not billable to the patient’s insurance company.

*Depending on insurance coverage, the patient may be required to pay out of pocket (insurance may not cover the entire cost of a visit). Billing department will reach out to the patient if any balance is the patient’s responsibility.

What I-693 form does your office use?

We use the most up to date I-693 form from U.S.C.I.S. 

Currently that is Form I-693 OMB No. 1615-0033 Expires 07/31/2025 Edition 07/19/22 including the COVID-19 Vaccination record.

Insurance: 

Why didn’t my insurance cover the whole costs?

Depending on insurance coverage, the patient may be required to pay out of pocket (insurance may not cover the entire cost of a visit). Billing department will reach out to the patient if any balance is the patient’s responsibility.

What is an ARNP?

An advanced registered nurse practitioner (ARNP) is a registered nurse who completes a graduate-level educational program. An ARNP can have primary responsibility for patient care.

The licensed ARNP may:

  • Examine patients and establish diagnoses by patient history, physical examination and other assessments
  • Admit, manage and discharge patients to and from health care facilities
  • Order, collect, perform and interpret diagnostic tests
  • Manage health care by identifying, developing, implementing and evaluating a plan of care and treatment for patients
  • Prescribe therapies and medical equipment
  • Prescribe medications when necessary
  • Refer patients to other health care providers, services or facilities
